OD 20/20 OS 20/20
Personal ocular history was unremarkable. She is currently taking medication for thyroid disease, hypertension, hypercholesteremia, GERD, and seizures
Slight near blur, dryness, redness, and foreign body sensation OU.
Vision with current spectacles was 20/20 OD and OS at distance and 20/20 OSD and OS at near. The patient has hyperopia and presbyopia OU, and low astigmatism OD.
Slit lamp exam was significant for pingueculae OU, palpebral conjunctiva papillae OU, arcus OU, and nuclear sclerosis. Pupil testing was normal. The patient's screening visual fields had areas of central depression and enlarged blind spots. Intraocular pressure was not measured. Upon fundus examination, a optic discovered at 7 o'clock OD. The C/D ratios were approximately 0.3 OU. Drusen was found in the macula OU, and retinal vessels exhibited some attenuation and tortuosity.
Due to the optic disc hemorrhage in the right eye, the abnormal screening visual fields for both eyes, and the lack of an intraocular pressure readings, the patients prescription was put on hold and she was referred to see an ophthalmologist for further evaluation.
